AACD Annual Smile Gallery Winners

The AACD Smile Gallery takes place at each Annual AACD Scientific Session. The event is open to AACD members, and provides an annual event to celebrate and share cosmetic dentistry. Below are the Best of Show and People's Choice winning cases from 2004, 2005, 2006 and 2007's Smile Gallery held in Vancouver, BC, Nashville, TN, San Diego, CA and Atlanta, GA respectively.

2007 Smile Gallery Winners

Best of Show

Dentist: Ted Murray, DDS

Evidence suggests that the illicit use of methamphetamine is on the rise. The oral manifestations of its use involve rampant deep caries with associated periodontal involvement. The social and financial costs of the proliferation of this drug are staggering. When faced with the opportunity to help a recovering user regain his self-esteem and position in society, the cosmetic dentist faces the challenge of how to best achieve the goals of maximum esthetics while utilizing minimally invasive methods that also allows for future maintenance should the need arise. The use of direct composite resin seemed the logical choice. After addressing the hygiene issues the teeth were restored using 4 Seasons Composite Resins (Ivoclar). The soft tissue was modified with the Odyssey Diode laser (Ivoclar) during the procedure. The lingual surfaces and dentin areas were restored with 4 Seasons dentin layered in different shades from cervical to incisal. Characterization was added to the dentin to mimic the adjacent teeth. 4 Seasons enamel was layered onto the facial, interproximal and incisal surfaces. Finishing and polishing was done with a combination of finishing burs, Astropol polishing points and Cosmedent’s Top Finisher system.

Dental Laboratory: Prowest
Dental Laboratory Technician: Chang Kim, MDT

This patient had ortho treatment when she was a dental student. The results of the ortho treatment did give her straight teeth, however, that was not an attractive smile for her because when she smiled she did not show her teeth. Her smile gave the impression that she had no teeth. One other concern for her was her occlusion. She always felt that when she was relaxed she had too much space between her molars. She needed to over-close her jaw to have contact and that position did not come naturally. The goal was to give her longer teeth for aesthetic and to open the bite for function. Empress crowns and veneers were used on front teeth #4 and #6-#13 (Tooth #5 was extracted before ortho treatment). Following that, Empress crowns were bonded onto the back teeth#2, #3, and #14 to support the new bite.

Dentist: Dr. Frank Stone
Laboratory Technician: John Haupt, MDT

Our esthetic goal for this patient was to transform her unsightly smile composed of opaque and ill-sized PFM's into a feminine, soft and natural smile that would "showoff" her central incisors and create proportionate tooth sizing and symmetry with respect to the lateral smiles and buccal corridor so that the overall esthetic would radiate a natural harmony between her face, lips and teeth.

The tricky part was managing dark endodontically treated teeth #7-10, uneven gingival heights across the maxillary anterior gingival margins and blunted papilla caused by splinting 8-9. By utilizing laboratory processed "transitionals", I was able to internally bleach and bond ceramic posts into #7-10, change gingival heights and artistically lengthen the incisal edges and allow time for tissue maturation before finalizing the finished margins for porcelain bonded restorations.

Laboratory Technician: Brad Jones
Dentist: Dr. Jeffrey S. Burns

Restorations on teeth ##3-13 and #28 and reshaping on ##22-27.
Restorative material used: Pulse BOO+ Ingot (Jensen Industries; New Haven, CT). The patient’s chief complaint was that her teeth were not attractive. She also exhibited with interproximal decay. Her lower anteriors were bleached and reshaped, and #27 was restored with a three-quarter crown. The anteriors were prepared as Class III, the upper bicuspids were prepared for three-quarter crowns, and tooth #3 was prepared for a full-coverage crown. All restorations were all-ceramic.

2005 Smile Gallery Winners

Best of Show

Dentist: Dr. Thomas Nabors
Laboratory Technician: Adam Adamonis

The patient desired replacement of crowns 8 and 9 (her two front teeth). After undergoing an exam and taking images, the treatment planned to replace the existing crowns with feldspathic crowns on 8 and 9, along with placing feldspathic veneers on teeth 7 and 10. The procedure also included soft tissue re-contouring and teeth whitening. The patient was fitted with temporaries for several weeks before placing her permanent new smile to help guide the final tissue margins. She used therasol rinse throughout the procedure and to aid in tissue healing and overall oral health. The treating doctor believes that cosmetic dentistry should achieve beautiful results like these while involving as few teeth as possible. This award winning case involved a lot of patience: waiting for the proper whitening results and tissue healing, as well as proper post-treatment care.




Peoples' Choice Award

Dentist: Dr. John Vallone
Lab Technician: Chang Kim, MDT

For this patient we placed ten Empress restorations on teeth #4-13. The goal of treatment was to realign the malpositioned teeth, improve the shade, and contour, eliminate the "gummy appearance", enhance the buccal corridors and feminize her smile. The patient didn't want to go through orthodontics as originally suggested. Gingival laser re-contouring was also performed using a diode laser on teeth #'s 7, 8, 10. The patient was warned of possible endodontic treatment on teeth #'s 6, 7, 9, 10, 11. Fortunately the case was completed without endodontic intervention. The laser re-contouring, tooth preparation, and the final impressions for all ten teeth were completed in a single day. All ten Empress restorations were cemented and the patient's mandibular teeth were bleached on the second appointment.

Best of Show

Dentist: Dr. Ralf Griesse
Lab Technician: Kent Hallmeyer, CDT

Treatment: This 17 year-old patient showed tetracycline staining and a diasthema between her incisors. She desired a closure of the gap between her front teeth, and a whiter, natural-looking smile. Treatment included ten porcelain veneers to change her teeth's color and appearence in the visible zone. The result is a lifelike, bright smile and a very happy patient.

Dentist: Michael Reece, DDS
Lab Technician: Young Kim, CDT

Treatment: Connie Wilson was a pain patient that was suffering from headaches and neck and shoulder pain. She was treated with a neuromuscular orthotic for 4 – 5 months until all of her symptoms had subsided. The doctor then placed 28 veneers on her new neuromuscular bite. Connie is now 8 months post insertion and is still symptom free.

Cosmetic Dental Procedures

Problem
Stained, discolored, or dull-looking teeth

 

Treatment Options
If you have stained or discolored teeth, but otherwise like your smile the way it is, all you may need are whitening procedures to give you the smile you've always wanted.

People with stained or dull teeth will benefit from whitening - a safe, effective means to whiten stained, discolored or dull teeth (or even a single tooth). In-office and supervised at-home whitening systems are available. At-home systems should be individually fitted and monitored by your cosmetic dentist.

Chipped, cracked, or worn teeth

 

Treatment Options
Teeth that are chipped, cracked, or worn can be perfectly restored with ultra-thin, specially made laminates or veneers. Either option can close gaps or disguise discolored teeth that do not respond well to whitening procedures.

Long-lasting porcelain or composite veneers are ultra-thin, specially made laminates that are bonded to the teeth. Veneers are stronger and have a longer life than bonding, but the process requires more time.

Bonding is a process in which an enamel-like material is applied to a tooth's surface, sculpted to an ideal shape, hardened, and then polished for an ideal smile. This procedure usually can be accomplished in a single visit.

Missing teeth or general bite dysfunction

 

Treatment Options
Missing teeth can be replaced with predictable results using fixed bridges or dental implants. Crown or bridge work is best for correcting major functional or structural problems with individual teeth, missing teeth, or general bite dysfunction.

New porcelain and ceramic technology is available to replace missing and/or severely broken teeth so that no one can tell that the replacements are not real.

Implants are being used to successfully replace teeth for people in all age groups-with esthetic and predictable results.

Excessive or uneven gums can be corrected with cosmetic surgery, a gum lift, resulting in a more even, pleasing smile.
